Field Hospital in Hearts of Iron IV

In plain English: what a Field Hospital is in HoI4, why you need it, who should take it, and how much it actually saves manpower and division experience.

What a Field Hospital is

In Hearts of Iron IV you build a division from pieces: infantry, artillery, support, and so on. A Field Hospital is a support company with medics. Its job is not to “hit harder than everyone else,” but to save the wounded, return some manpower to the pool, and burn less division experience in hard fights.

In the designer it sits under the Support Companies tab. It does not add combat width. You can have one company of this type per division.

Why you need a Field Hospital

Because fights burn manpower and experience. Without medicine, losses vanish into nowhere, and the division slips toward green recruits more often. The hospital plugs that gap.

  • Casualty Trickleback. Some of the wounded return to manpower — the country lasts longer in the war on the same pool of people.
  • Fewer experienced soldier losses. The division sheds less experience when it takes casualties — it stays trained and sharp longer.
  • Slightly tougher infantry. The hospital gives an HP bonus to all infantry in the division — the template holds a hit a bit better.
  • No width. Saves manpower and experience without bloating combat width.

This is not artillery and not engineers: the hospital barely raises damage and does not dig trenches. Its value is in manpower, experience, and infantry toughness.

When and who needs a Field Hospital

  • Countries with a small population and manpower — almost always. If every soldier counts, the hospital is one of the most useful support slots.
  • Divisions that fight often. The more fights and losses — the more Casualty Trickleback and experience saving pay off.
  • Frontline infantry templates. The infantry HP bonus sits on infantry battalions — pure tanks with no infantry get less out of it.
  • Early game with a huge manpower pool — case by case. If manpower is endless and slots are few — engineers, artillery, or logistics are sometimes more important.
  • Garrisons and rear — usually no. If the division barely fights — better not spend Support Equipment and trucks.

Note: you do not need to put a Field Hospital on every division in HoI4. It is relatively expensive, especially for minors. Put it only on large offensive infantry divisions.

How to get a Field Hospital into a division

Three steps:

  1. Unlock trucks and research Field Hospital I. First you need the Motorized Infantry tech (trucks). Then in the support company tree unlock Field Hospital I. Research II and higher after that — Casualty Trickleback and experience saving grow.
  2. Set up production. The company needs Support Equipment and trucks. Without them in stockpile, the template will be understrength.
  3. Add it in the designer. Spend army XP to put Field Hospital into a Support Companies slot. Then divisions on the map draw equipment from the stockpile.

How effective a Field Hospital is

The hospital’s value is in manpower, experience, and a bit of infantry toughness — not damage. On long wars and for countries with a thin manpower pool it is one of the most “worth it” support slots.

Where it shines

  • Manpower savings. Some losses return — the war lasts longer on the same pool.
  • Keeping experience. Divisions slip toward green recruits less often after meat grinders.
  • Infantry templates. The HP bonus to infantry shows up most on the line.
  • Slot cost. No width; needs Support Equipment and trucks, but the effect stacks over months of fighting.

Where it is weak

  • Damage and entrenchment. Barely fights and does not dig in. For combat you need artillery, anti-tank, engineers.
  • Needs trucks. Without motorization and a truck stockpile you cannot field it properly.
  • On a quiet front. If the division barely takes losses — less value than combat slots.
  • Does not fix supply. If the division is starving — logistics first, not only medics.

How this looks in practice

  1. Research Motorized Infantry and set up truck production.
  2. Research Field Hospital I and Support Equipment production.
  3. In the designer, add Field Hospital under Support Companies on divisions that fight often.
  4. Then keep engineers, artillery, logistics as needed — the hospital usually stays where the meat grinders are.

Practical rule: infantry holds, engineers help them stand, artillery pushes, and the Field Hospital makes sure the country and the division do not burn out on manpower and experience.

Tips

  • Trucks first. Without Motorized Infantry and a truck stockpile you will not field the hospital properly.
  • Put it where they fight. Frontline combat divisions — higher priority than garrisons.
  • Do not expect artillery from it. It is about manpower and experience, not soft attack or entrenchment.
  • Research the hospital branch. The same slot returns more wounded and protects experience better with newer techs.

Bottom line

Unlock it via Motorized Infantry and Field Hospital I, feed it Support Equipment and trucks, put it in Support Companies.
